In January 2023, UnitedHealthcare Community & State launched a partnership with Waymark to improve access to clinical and social care for 15,000 members enrolled in Medicaid in Washington state. The goal of the partnership is to enable primary care providers (PCPs) to extend their reach beyond the clinic through community-based care teams comprised of community health workers (CHWs), licensed therapists, and care coordinators. These teams use proprietary data science technologies to proactively identify patients at the greatest risk of poor health outcomes and match them to the best evidence-based intervention.

Today, we’re excited to share the results of our partnership — including reduced non-emergent ED visits and inpatient admissions, early improvements in HEDIS scores, and increased patient and provider satisfaction.
